Who We Are

Dr. Zylinski

was born in Buffalo, NY and grew up in nearby Lake View, NY (Go Bills!).  He attended the University of New England College of Osteopathic Medicine (Class of 2018), followed by the St. Elizabeth Family Medicine Residency Program (Class of 2021), followed by some additional training at Michigan State University in osteopathic neuromusculoskeletal medicine (Class of 2022) before returning to the Utica area to practice.  He started working at The Falcon Clinic in November 2022 and assumed ownership of the practice on March 1, 2025.  He is very passionate about providing high quality health care with a distinctively osteopathic flavor to patients from all walks of life.  He is also actively involved in teaching medical students and residents.  He is also a medical officer in the Army Reserve.  Other interests/hobbies of his include ice hockey, guitar, and church activities.  He currently lives in New Hartford with his wife, Sarah, and dog, Kenobi.

Dr. Chmielewski

Dr. Richard Chmielewski was raised in a Polish-American immigrant family, spending most of his early life in West Utica.  He attended Utica area schools and Utica College.  He did spinal cord research at Cornell University and graduated from the College of Osteopathic Medicine and Surgery in Des Moines, Iowa in 1976.  He practiced emergency medicine in Central New York for almost 30 years and was one of the first physicians board certified in that specialty until his retirement from that.  He is currently board certified in neuromusculoskeletal medicine/osteopathic manipulative medicine (NMM/OMM).  He is the Founder of the only premier osteopathic general medicine practice in the area, The Falcon Clinic for Health, Wellness, and Recovery, where he continues in active practice with one other physician.

He and his wife, Paula, and a handful of friends started EMS Global, Inc., a nonprofit organization, in 1994 to assist in the modernization of emergency medical care in the country of Poland.  He was instrumental in promoting and teaching American Heart Association courses on resuscitation throughout Poland from 1995 to 2008.  As a practicing emergency medicine physician, he was made the Ambassador to Poland by the American College of Emergency Physicians.  In 2000, at the first International Conference on Emergency Medicine in Wrocław, Poland, he was the first among two physicians to be granted Honorary Membership into the Polish Society of Emergency Physicians.

He is a novelist, songwriter, musician, medical researcher, and lecturer and is presently a clinical instructor for several osteopathic medical schools in the Northeast.  He is proficient in a handful of foreign languages, including Polish and German.
