The Falcon Clinic
The Falcon Clinic for Health, Wellness, and Recovery (“The Falcon Clinic”) was founded in 2004 by Richard Chmielewski, D.O. Prior to then, Dr. Chmielewski served as a board-certified emergency medicine physician for over 20 years at multiple hospitals in the Central New York region. However, he realized that something was missing in our community: a bona fide osteopathic general medicine practice that offered primary care, osteopathic manipulative treatment (OMT), and addiction treatment to patients based on the unique and still resounding philosophy and principles of Andrew Taylor Still.

Over the next 20+ years, the clinic has changed several providers and locations, eventually ending up at its current location at Oxford Crossing in New Hartford. However, our commitment to providing high quality health care with a distinct osteopathic flavor to patients from all walks of life is still alive and well and stronger than ever.
Brian Zylinski, D.O. started working at The Falcon Clinic in November, 2022 and assumed ownership of the practice on March 1, 2025. Dr. Chmielewski continues to work for the clinic as an independent contractor under Dr. Zylinski (and no, he has no plans on retiring any time soon!). Together, with their staff, they represent Central New York’s premier osteopathic general medicine practice.
Here at The Falcon Clinic, we work very closely with George Kozminski, M.D. Dr. Kozminski has a separate practice from us and rents space from us.

We endeavor to deliver care of the highest quality and with a distinctively osteopathic flavor to patients hailing from all walks of life. The providers in our office are constantly pursuing continuing educational experiences in order to gain new knowledge and skills to benefit our patients. We have also partnered with various academic institutions and community organizations in order to increase our impact on the local, regional, national, and international levels. Long-term goals of ours include (but are not limited to) franchising and publishing groundbreaking osteopathic research. At The Falcon Clinic, we are forever grateful to all of our patients who entrust us with the gift and pleasure of their care.

Treatment Statement
The Falcon Clinic for Health, Wellness, and Recovery (“The Falcon Clinic”) is a primary care clinic located in New Hartford, NY. We offer general medical care with a major focus on osteopathic principles and on prevention and treatment of behaviors that are destructive to health. We offer primary care treatment for many medical conditions, including (but not limited to) hypertension, diabetes, thyroid disorders, skin diseases, stomach and intestinal problems, neurologic disorders, and sports injuries. We also treat minor injuries, wounds, and small lacerations. We do not accept any form of Workers’ Comp or No Fault coverage.
Our physicians are fully trained and licensed to provide manual treatments of acute and chronic musculoskeletal problems, including (but not limited to) neck pain, back pain, arthritis, and fibromyalgia. As an osteopathic medical facility, you will get a full medical evaluation from an osteopathic physician prior to any manual treatments.
Abuse and improper prescribing of antibiotics has fostered an epidemic of antibiotic resistance. Similarly, abuse and improper prescribing of narcotics and other addiction-prone medications has fostered an epidemic of chemical dependency. The Centers for Disease Control (CDC) has issued multiple statements to providers urging them to reconsider their liberal antibiotic prescribing practices, seeing as most infections are viral and self-limiting in nature. Similarly, the New York State Department of Health (NYSDOH) is monitoring and urging providers to limit the use of addiction-prone medications, including (but not limited to) opioids, benzodiazepines, sleeping pills, and stimulants. We wholeheartedly agree with these efforts.
Patients coming to our practice must understand that we do not indiscriminately prescribe antibiotics. In addition, narcotics and other addiction-prone medications are only prescribed short-term and for very specific reasons. Any long-term prescriptions for these types of medications need to come from a specialist, such as pain management or psychiatry.
